When enabled, the website form collects your name, email address, optional phone number and preferred contact method so the practice can respond to a general visit request. It does not request health history or clinical details. Please use the practice’s designated intake process for sensitive information.
Delivery and retention
The form emails the configured practice inbox and does not create a separate patient database. Website hosting and email providers may retain delivery records and technical logs. The practice should confirm its actual providers, retention practices and privacy contact before launch.
Website protection
A security token, a hidden spam field and temporary hashed-IP rate limits help protect the form. These protections do not turn ordinary email into a clinical records system.
External services
The location page embeds Google Maps, which contacts Google. External services have their own privacy policies. No advertising or analytics is installed by this package. The introduction video is served locally.
Questions and choices
Contact the practice directly about access, correction or deletion of general inquiry information. A verified privacy contact must be added by the practice before the website is launched.
